BACKGROUND
In February 2018, the San Pablo Police Department began the P.A.T.H. (Partnership in Assisting the Transition from Homelessness) Program due to a larger number of people experiencing homelessness in Contra Costa County, specifically in the City of San Pablo. PATH was an impermanent program with the project goal to identify resources and scope of work needed to improve the quality of life for community members by engaging individuals and families experiencing homelessness and leveraging service-oriented strategies.

On October 2, 2018, City Council adopted a Resolution declaring a shelter crisis in San Pablo pursuant to SB 850 (Resolution #18-342).

Following this City Council action during 2018, the San Pablo Police Department was invited by Kaiser Permanente to apply for the Kaiser Permanente Community Benefits Program Grant for $60,000 to support a Regional West Contra Costa County CORE (Coordinated Outreach Referral, Engagement) Team for the cities of San Pablo and R...
