BACKGROUND

Recently, Lifelong Medical Care, Inc. (Lifelong) leadership requested a letter of support from the City of San Pablo for Lifelong’s Service Area Competition (SAC) application (HRSA-20-015) to the US Health Resources and Services Administration of the U.S. Department of Health and Human Services.  This federal grant funding program, if awarded to Lifelong, would enable continuation of its Section 330 funding to assist Lifelong to continue serving low-income and vulnerable populations in Alameda County, Contra Costa County and Marin County, California.

 

A general outline of the SAC HRSA-20-015 Funding Program is provided below:

Eligible Applicants:

Public or nonprofit private entities, including tribal, faith-based, or community-based organizations.  See Section III.1 of this notice of funding opportunity (NOFO).

 

Currently, the City of San Pablo owns a building, located at 2023 Vale Road in San Pablo, which houses the Lifelong Brookside San Pablo Health Center.  This Lifelong facility provides high-quality medical, dental, and social services to people of all ages. Lifelong is dedicated to helping each patient live a healthy productive life. In addition to offering care, Lifelong offers a range of patient education and outreach services.

 

Patients of LifeLong Brookside San Pablo Health Center have access to a wide variety of providers and resources. Lifelong is continually evolving to meet the needs of local patients and the surrounding community.  Lifelong’s core services include:

 

                     Primary Health Care for Adults

                     Pre/Post Natal Care

                     Pediatrics

                     Case Management

                     Multi-disciplined Care Coordination

                     Health Education

                     Social Services Resources

                     Patient Advocacy

 

City Council Ratification Approval

On July 22, 2019, a letter of support was drafted and signed by Mayor Kinney to be transmitted to CEO Marty Lynch of Lifelong Medical Care in support of its SAC HRSA-20-015 grant application (see attachment). 

 

Ratification of said letter of support requires City Council minute action approval via majority vote this evening.
